To help our uninsured patients, we designed an affordable alternative to maintain dental care. Our In-House Dental Care Plan is an opportunity to visit us regularly, and you can have Bella Smile quality of services by signing up for our annual plan.

We believe everyone should have access to great and affordable dentistry, and preventive care is essential for long-term oral health, so this membership plan aims to be another viable payment option for our existing and new patients.

What’s included (per year):

  • 2 (two) Consultation (any type) with the doctor;
  • 2 (two) Regular Cleanings (Prophylaxis Cleaning or Periodontal Maintenance);
  • 1 (one) Full Mouth Series of X-Rays
  • 1 (one) Panoramic X-Ray;
  • 1 (one) Zoom take Home Whitening w/ customized trays.
  • 15% off on all restorative procedures
  • All services are valid for 1 (one) year starting on the date of purchase.

Terms & Conditions:

  1. This program is a discount plan, it is not a dental insurance and it cannot be used in conjunction with other insurance plan.
  2. This program is exclusive at Bella Smile office.
  3. Non-Transferable, Family members cannot be substituted for another family member.
  4. Dues are Non-Refundable
  5. Coverage needs to be used within the year of enrollment and cannot be carried over into the following year.
  6. All Payments are due at the time service rendered in order to receive the discount.
  7. Membership payment to be received in full in order this is a provision of the plan.

  • Adults (16 years old and older): $299 (per person and per year)
  • Children (15 years old and under): $199 (per person and per year)
