Terms & Conditions:

  1. This program is a discount plan, it is not a dental insurance and it cannot be used in conjunction with other insurance plan.
  2. This program is exclusive at Bella Smile office.
  3. Non-Transferable, Family members cannot be substituted for another family member.
  4. Dues are Non-Refundable
  5. Coverage needs to be used within the year of enrollment and cannot be carried over into the following year.
  6. All Payments are due at the time service rendered in order to receive the discount.
  7. Membership payment to be received in full in order this is a provision of the plan.
